Patients living within the practice area are welcome to register and you can confirm your eligibility from your postcode, simply click on the tab Practice Boundary and put in your postcode. You are required to bring along proof of address such as a recent utility bill when you attend the surgery to sign your registration. 

You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You are advised to ensure that until full registration is complete that you have enough of any medications you are taking from your previous practice as we will not be able to prescribe until you have been seen by one of our GPs.

Before your registration is complete you are required to present at the surgery proof of id such as driving licence OR birth certificate OR passport AND proof of address. Medical treatment is only available after full registration.
